Saint Agatha Foundation awards grant funding to our partners in care, who then work directly with patients to approve and distribute financial assistance. For most patients receiving support, the nonprofit organization CancerConnects serves as the connection between the partner provider and our Foundation.

That support can help cover treatment, procedures, and testing, prescription co-payments and medications not covered by insurance as well as office visit co-payments; transportation to and from treatment; and non-medical living expenses during active care.

CANCERCONNECTS APPLICATION PROCESS

Step 1: Get your application

Download an application for Saint Agatha Foundation assistance from CancerConnects, or call their office at
(315) 634-5004 and request a copy.

Step 2: Complete your sections of the application

Fill in your patient information, diagnosis, insurance, and the specific expense or amount you’re requesting help with. You’ll also need to include proof of income (most recent tax return, or a benefits statement if you don’t file taxes).

Step 3: Have your care team sign the provider section

Your current treatment provider needs to complete and sign a short attestation confirming your treatment status.

Step 4: Submit your application

Mail your completed application to CancerConnects, or email a scanned PDF copy. They cannot accept photos of the applications or supporting documents.

Step 5: Know what to expect next

Applications are reviewed on a set schedule, and processing can take a few weeks once everything is received. CancerConnects handles your application directly, so they’re your best point of contact for a status update. If you don’t hear back for a while, it’s always okay to call and check in at:

Recovery & Everyday Support

Saint Agatha Foundation also partners with organizations across Central New York that provide recovery and wraparound support, including including child care, counseling, transportation, legal assistance, and other everyday needs that come up during and after treatment. Camp Kesem is a nationwide community, driven by passionate college student leaders, supporting children through and beyond their parent’s cancer diagnosis and treatment. The Syracuse area camp is held through Syracuse University.

Francis House provides a home and a supportive extended family to people with terminal illnesses with a prognosis of six months or less to live. Saint Agatha Foundation funding provides support for those with breast cancer.

Legal Services of Central New York is an organization advocating for low-income families and individuals, and providing free, civil legal assistance in thirteen counties of Central New York. In partnership with the Saint Agatha Foundation, their Cancer Legal Advocacy & Services Project  offers free civil legal services to breast cancer patients and their families.
